Wilford Hall ECMO Team Helps Save Baby

LACKLAND AIR FORCE BASE, Texas - A crew of Air Force and civilian medical specialists flew to Honolulu

July 21 to transport a 12-day-old baby girl to Rady Children's Hospital in California for advanced medical

care.


The 11-member critical care air transport team, which included a neonatal critical care physician,

neonatology fellows, neonatal intensive care nurses, and respiratory therapists along with a pediatric

intensive care nurse, boarded a C-5 Galaxy at Kelly City USA, Texas, with their battery-operated portable

ECMO equipment. ECMO, or extra corporeal membrane oxygenation, is a heart-lung bypass device which

circulates and oxygenates the blood, giving diseased or damaged lungs a chance to heal.


The shunt that was keeping the then 10-day-old infant suffering from a congenital heart defect alive failed.

She needed care and treatment that was not available in Hawaii. A request for help to get her to San Diego

was sent to the 59th Medical Wing's ECMO team.


"As soon as everyone realized the life of a 12-day-old infant hung in the balance, it stopped being just a

normal duty day and turned into the mission of a lifetime," said Maj. (Dr.) Melissa Tyree, medical director

ECMO and neonatal intensive care unit transport services at WHMC.


Despite a delay due to engine trouble on the KC-135 Stratotanker, the team still was able to get to the C-17 Globemaster III that was waiting for them in Hawaii on time and complete the transfer from the Kapiolani

Medical Center in Honolulu July 22.


"The 167th Aeromedical Evacuation Squadron worked tirelessly to off-load our 2,000 pounds of equipment and expeditiously transferred us from the grounded KC-135 to the mobilized C-5 with temperatures as high as 110 degrees on the flightline. The 433rd Airlift Wing scrambled to put together a last-minute mission to get us to Hawaii in time to still board the C-17 from the 535th Airlift Squadron awaiting us at Hickam Air Force Base," Dr. Tyree said.

After landing in Hawaii, the WHMC ECMO team completed a delicate procedure lasting six hours to move

the infant from the hospital's life support system to the team's portable system.


The team continued to monitor and treat the baby during the flight to San Diego."The 349th AES crew and

C-17 pilot took off and landed with such precision and stability that it was nothing short of perfection,

" Dr. Tyree said.  As the team left Miramar Naval Air Station headed toward Rady Children's Hospital in a

caravan of ambulances with blazing lights and sirens, the baby's mother expressed through tears of joy that

she was most moved by the fact that so many people from across the United States came forward to save

her baby's life, said George Brandon, pediatric intensive care unit nurse, 59th Medical Inpatient Squadron.


Still on the WHMC's portable ECMO circuit, the baby was taken for surgery July 24. Once in the operating

room, she was transferred to its bypass circuit and underwent a successful shunt replacement.

"The infant is now stable in the intensive care unit off ECMO and doing very well," Dr. Tyree said.

No other hospital has global air transportable ECMO capability. Wilford Hall staff place four to eight

patients on ECMO each year, and two to four require long-distance transport.

 Advanced aeromedical services for Neonatal and ECMO transports are two of the new elements that we will be offering in 2008.
Below are some pictures of the ECMO - Extra Corporeal Membrane Oxygenation transport equipment that we will be placing aboard a Certified Aeromedical Lear 35. 
 
Investors (MD's, RN's and RT's) who have voiced interest in participating in these operations will be given an opportunity to get in on the ground floor.  Investment proposals are now being taken and reviewed; we have uniquely targeted the Neonatal and Pediatric specialties since part of the operations will requiere their actual hands on operations. 
 
Why ECMO?   Because with only the US Air Force at Wilford Hall Medical Center has had the capability to accommodate the large structure in C-130's or larger aircraft at an outrageous cost to taxpayers.  Our aeronautical engineer designed a special unit that has been certified by the FAA (STC'd) exclusively for the Lear 35 series, an aircraft that lends itself to long range missions; it  has over 2000 miles of non-stop range, a wide cargo door and quiet engines that perfom better at higher altitudes (up to 41,000 feet).
 
We have been invited to several events beginning february 2008 and we expect to be able to attend some if not all, in order to demonstrate our new capabilities in Air Ambulance Diversification.

TRASLADOS DE NEONATOS CON SERVICIO DE OXIGENACION MEMBRANAL EXTRACORPOREA
 
ECMO por sus siglas en Ingles, es una modalidad que exige una altisima dedicacion al aprendizaje y manejo de este tipo de pacientes.
 
Tanto la maquinaria como la sensibilidad del cuerpo medico, influyen mucho en los resultados de un recien nacido con complicaciones cardiacas o cardiorespiratorias.  Se ha usado tambien en casos de jovenes y adultos, en forma estatica hospitalaria; solo qe ahora podemos ofrecer un servicio internacional de traslados de recien nacidos y de pacientes pediatricos hasta de 7 años que requieran este servicio.
 
Para Ello, estamos formando un grupo de destacados profesionales en el ramo de la neonatologia avanzada que formaran parte del cuerpo operativo, administrativo y educativo de este programa que no tiene precedentes significativos solo la fuerza Aerea de Estaods Unidos quienes han sido la unica entidad que ha colocado sus expertos y los aviones de maximo alcance (como el Hercules, el C-17, C150 Y otros) para hecer estos translados.  Lo que es controversial es el costo asociado con estos traslados.  Pero los centros pediatricos y especializados en este programa, cooperaran con autorizaciones de pago a nuestra empresa.  Tenemos tambien el apoyo de los directores del programa ECMO en la Base Militar Medica "Wilford Hall Medical Center" en Texas, donde residen el centro principla de los traslados ECMO militares.
 
En el 2005, La Universidad de Arkansas y el Centro Medico asociado con la U., equiparon un Lear 35 con una camilla especializada pero que debido a su inversion, unicamente proveen el servicio a pacientes que llegan a su hospital.  Lo que limita ese servicio a pocos y no es obtneible a ningun precio.  Igualmente la Universidad de Michigan, equipa aviones, con el mismo proposito.
 
Lo que deja un amplio campo para un servicio nacional e internacional sin ninguna razon que impida prestar el servicio a quienes lo necesiten.
 
Nos han llegdo ofertas de aviones, de medicos, de euipos asociados a ECMO y es hora de revisar propuestas para iniciar este servicio lo mas pronto posible.  Inversionistas que tengan la vision que ECMOJet tiene, se invitan a participar en el desarrollo de esta modalidad, de inmediato.
 
Buscamos Neonatologos, enfermeros/as y perfusionistas bilingues que tengan o la certificacion adecuada o la forma de invertir en este concepto.
 
Empresarios que deseen invertir en la idea deben tener mas que interes...deben formar parte de un conjunto de ideas que van a florecer de inmediato.

Como ordenar nuestro Servicio Aeromedico

 

Cada vuelo aeromedico tiene un costo diferente y seria imposible dar un precio ya que el tipo de avion, condicion del paciente, elementos medicos, paises de entrada y salida y otros costos no darian una idea exacta.

 

Para poder hacer una cotizacion, los siguientes informes deben incluirse:

  1. Nombre, edad y sexo del paciente
  2. Nombre, direccion y telefono de Medico de Cabacera
  3. Lugar de residencia o internacion del paciente (Nombre, direccion y telefono directo a enfermeria).
  4. Diagnosis y fecha de internacion
  5. Equipos requeridos (Ventilador, cuna incubadora,, etc.)
  6. Aeropuerto mas cercano con servicios de combustible, rescate y areas de descanso.
  7. Nombre, direccion y telefono de servicios de Ambulancia Terrestre aun si el hospital o la familia hacen arreglos de envio.   Nuestra tripulacion saca al paciente de un hospital y lo entrega en el otro asi en muchos casos solo se requiere una ambulancia basica.
  8. Metodo de pago:  Tarjeta de Credito VISA o MC. No se acepta American Express.  O deposito directo a nuestro banco en dolares USD.

 

Nuestro servicio se basa en las reglas del Estado de California, la FAA y la OACI.

 

El componente de tripulacion es de Enfermero TAC  (Tripulante Aeromedico Certificado)  y combinacion con enfermero, paramadico o Tecnico de Inhalatoria. 

 

Medicos acompanantes no se aceptan a menos que tengan designacion TAC y remplacen uno de nuestros tipulantes.

 

Se utiliza el sistema ACLS para TODOS tipo de traslados no importa que tipo de paciente requiera el servicio.

 

Vuelos nacionales (USA) se despachan en termino de dos horas.  Internacionales requieren minimo de 24 a 48 horas dependiendo del pais y la localidad.

 

UN SERVICIO  AEROMEDICO QUE SEA MIEMBRO DE ALSAM EN LATINOAMERICA,  PUEDE SER CONTACTADO PARA UNA DERIVACION.  TRIPULANTES TAC, EQUIPOS APROBADOS POR ALSAM Y ACREDITACION ES UN REQUISITO PRINCIPAL PARA QUE LA RED INTERNACIONAL FUNCIONE CON OPTIMA CALIDAD.
